Empowering Conversations About the Future

Planning for the future can be daunting, even more so when living with dementia. Yet, having meaningful conversations about values, wishes, and what matters most, is essential. That’s why we’re proud to introduce Me and My Future; a toolkit designed to support these important discussions with individuals and in group settings.

Contents of the Me and My Future toolkit

A Gentle, Person-Centred Approach

Me and My Future helps people living with dementia reflect on what’s important to them - now and in the future. Whether used one-to-one or in groups, the toolkit creates a safe and supportive space for individuals to express their hopes, preferences, and choices in their own words. An AgeUK group facilitator shared:

“It really helped me get to know a lady and what’s important to her - home! The reminiscence of thinking back to her mother’s baking in the kitchen… it can help us plan activities that truly matter to her.”

Why It Matters

Conversations about the future don’t just help with care planning. They build understanding, strengthen relationships, and give people living with dementia a stronger voice in shaping their own lives. As one person put it:

“It’s been refreshing to start these conversations… I took away an ‘open door’ that’s always been shut.”

Co-Designed With Lived Experience

This toolkit was born out of a need identified in 2020 through the Sheffield Dementia Partnership Meeting, led by Michelle Wilde from Dovercourt Group Practice, Sheffield. From the start, the design process has involved people living with dementia, families, dementia link workers, occupational therapists, and community partners. Through workshops, pilots and iteration, the toolkit has grown into a resource shaped by real voices and real experiences.

In 2025, following further testing with partners such as Age UK, Compassionate Sheffield, SACHMA, Darnall Wellbeing and SOAR, the toolkit was finalised.

Next Steps

Me and My Future is now available. We’re excited to share it more widely, work with partners to make it accessible to as many people as possible, and continue building a future where everyone can have their voice heard.
